Parent Committee (PC) Application

Parents of OHS students involved in an OHS Sports Team, Activity or Club can apply to OHS PTSA to become a Parent Committee. By doing so, the parents agree to follow and abide by all our rules and regulations. A PC must re-apply each year to continue to be a committee under OHS PTSA.

Fundraising Form

Before you begin fundraising, it needs to be approved by the PTSA Board.  Please complete the online form.

Disbursement Form

All disbursements from the account requires a disbursement form with the appropriate approvals.
